Pre Admission
Your Surgeon or Gastroenterologist will have booked you into Glengarry Private Hospital and given you admission forms to complete and send to Glengarry Private Hospital at least 48hrs before your admission date. This allows clerical staff time to prepare your notes prior to admission and address any concerns.
Once completed these forms can be posted to 53 Arnisdale Road, Duncraig 6023. Please ensure forms are posted at least five days before your admission. Alternatively you may wish to fax or hand deliver as follows:
